You must be an active benefit eligible employee to receive this incentive. If your spouse is enrolled and has Ball State insurance as their primary health insurance, they are also eligible.
Yes. Each program is independent of one another. You can receive one without receiving the other. All benefit eligible employees are able to participate in the LiveWell incentive. In order to participate in the BeWell incentive, you must be enrolled in the Ball State's medical insurance program through Anthem.
Each Incentive program starts on November 1st and will end on October 31st.

After completing the LiveWell Incentive Confirmation and Voucher Selection form, an email verification will be delivered to your BSU account within 24 hours.
No, all vouchers must be redeemed before their expiration date as there will be no extensions provided beyond the expiration date on the voucher.
To redeem your LiveWell incentive voucher, simply visit your selection and tell them you would like to redeem your voucher from Working Well. You may be required to show photo ID.

You must be enrolled in Ball State's health insurance plan to be eligible for the incentive. * As of November 1, 2021, spouses are not eligible for the BeWell incentive.
Have your physician's office fill out the Annual Physical Verification Form and return it to the Employee Solution Center (AD G004) by November 1, 2024.
