Our goal in Working Well is to create a vibrant culture of wellbeing across campus while rewarding participants for leading engaged and meaningful lives.

Eligibility

You must be an active benefit eligible employee to receive this incentive. If your spouse is enrolled and has Ball State insurance as their primary health insurance, they are also eligible.

  • Recreation Services (Student Recreation & Wellness Center, Auxiliary Services including Ball Gym
     $180 can be used for any services or fees offered by Recreational Services
  • Adult Physical Fitness Program
    $150 towards APFP membership
  • Emens Auditorium Performances
    $90 can be redeemed for any services offered by Emens
  • Ball State Athletic Tickets
    $90 can be redeemed to purchase Athletic game tickets
  • Outdoor Pursuits
    $90 can be redeemed for trips or equipment rental

The Ball State employee must log into bsu.lifework.com. Go to their profile button, select "Profile",  and select "Family." The employee will enter the spouse's email address, and LifeWorks will send an email invitation to join. 

Yes. Each program is independent of one another. You can receive one without receiving the other. All benefit eligible employees are able to participate in the LiveWell incentive. In order to participate in the BeWell incentive, you must be enrolled in the Ball State's medical insurance program through Anthem.

Each Incentive program starts on November 1st and will end on October 31st.

All vouchers will expire 1 year after issue date.

No. All purchases are final. No refunds or credits are available.

Participants are responsible for any expense greater than the face value of the voucher.

Yes, you will receive  $180 and may use it for a membership and a fit pass, just make the SRWC aware of your choice upon redemption.

After completing the LiveWell Incentive Confirmation and Voucher Selection form, an email verification will be delivered to your BSU account within 24 hours. 

No, all vouchers must be redeemed before their expiration date as there will be no extensions provided beyond the expiration date on the voucher.

To redeem your LiveWell incentive voucher, simply visit your selection and tell them you would like to redeem your voucher from Working Well. You may be required to show photo ID.

Working Well vouchers will be treated as taxable income for employees. Every employee who accepts a voucher will have the value of those vouchers included as income on their W2. 

Whether you perceive yourself to be healthy or ill, establishing a relationship with your family physician is very important. 

Eligibility

You must be enrolled in Ball State's health insurance plan to be eligible for the incentive. * As of November 1, 2021, spouses are not eligible for the BeWell incentive.

What You Need To Do

  • Complete your annual physical with your primary care physician before October 31, 2024
  • Access an account in Telus Health. Setup your profile to add as much information as you would like.

How to Prove You Completed Your Physical

Have your physician's office fill out the Annual Physical Verification Form and return it to the Employee Solution Center (AD G004) by November 1, 2024.

When Will You Receive Your $50 Reimbursement?

After completing necessary steps, you will receive an extra $50 in your December 2024 paycheck.

Yes. According to Internal revenue code, the university is required to report taxable income for all financial incentives that a faculty/staff member receives. The amount will be added to taxable earnings for employees.
