Members dealing with ongoing health conditions like asthma (pediatric and adult), diabetes (pediatric and adult), coronary artery disease (CAD), chronic obstructive pulmonary disease (COPD) or heart failure can sign up for ConditionCare, where you can get free one-on-one assistance from a registered nurse who can help manage symptoms so they have less impact on you and/or your family’s life.
Participation does not replace your doctor, but will work with him or her to provide you with the best health care possible.
