To complete your application, follow the instructions listed below. Please note the steps listed will all be within the CSDCAS application portal.
 
APPLICATION INSTRUCTIONS
  • Complete the CSDCAS application.
  • Statement of purpose: upload a personal statement in the Documents section of CSDCAS. The statement of purpose should include additional information to the AuD Admissions Committee about audiology as your career choice, long-term career objectives, and educational and experiential preparation for an AuD program. The statement must be typed and should be no more than two single-spaced pages.
  • Two letters of recommendation: letters of recommendation must be sent through the CSDCAS recommender portal.
  • Transcript/credential evaluation showing complete academic history: official transcripts from all U.S. institutions attended must be sent to CSDCAS.
  • International Applicants: request that official evaluations of transcripts from all foreign institutions be sent to CSDCAS as follows:
    • Use the World Education Services (WES) link in the Academic History section of the application to order your official WES evaluation or
    • Use the "Download Evaluation Request Form" in the Academic History section of the application to request an evaluation from an evaluation service other than WES.

OTHER ADMISSIONS REQUIREMENTS 
  • 3.0 GPA or higher
  • Bachelor's degree or higher from an accredited university

ACCEPTING ADMISSION
Admitted students must accept their admissions by confirming their enrollment via the Decision Reply Form available on the Cardinal Landing portal by May 1. 

PROGRAM INFORMATION
You will participate in clinical observation as a requirement of this program, but we encourage you to observe an audiologist before applying. Having observation hours will not impact your chances of being admitted to the program, but it may help you decide whether pursuing a doctoral degree is the right career choice for you. You may also use existing observation hours to meet the program requirements after you are admitted. If you have observation hours, please provide written documentation that includes:
  • the date
  • number of hours
  • a brief description of the procedures observed
This record must be signed by the audiologist you observed or the audiologist in charge of the observation. 

Cardinal Landing Portal

Upon submission of the application form and all required materials, you will receive an email with information on how to log into your Cardinal Landing Portal to view your admissions decision when it becomes available.

International applicants to on-campus programs will be required to submit all application materials by the earliest of two deadlines: either the international application deadline or the graduate program deadline. Applicants to online-only programs must submit all application materials by two weeks before the start of term.

Your application must be completed at least two weeks before the start of the semester. Start of term dates can be found on our Academic Calendar. Some graduate programs have priority application deadlines. Please review your program of interest's requirements for more details.


After you apply 

International Graduate Students

Beginning Fall 2026, all on-campus, degree seeking students admitted to Ball State must confirm their enrollment by replying to the International Decision Reply Form in their Cardinal Landing portal. Please review our 5 Steps to Admission for detailed information on enrollment and arrival planning.

From the Cardinal Landing Portal, admitted students can accept their admissions offer via the Decision Reply Form, review their next steps to enrollment, and learn more about orientation. Admitted students must accept their offer of admission via the Decision Reply Form by the Thursday prior to the start of the term.
