This year Ball State is requiring all participating children to have a signed medical release form for emergencies on file for each child. Both forms will be available to sign on the first day of classes. But if desired, this form can be downloaded, and the signed paper copy brought on the first day of classes to save time. Please do not send by email. The video and photography release form is optional, but you can bring that signed as well.
