The following documents give you information about the Banner HR module, including instructions for an Electronic Personnel Action Form (EPAF) position number, date, and error messages. Some links take you to text documents such as EPAF user guides, EPAF FAQs, Summer Assignments and the Banner Position Numbers with FOAP, which is a searchable PDF document updated monthly. An EPAF is used to create or update employee records in Banner. The links listed below provide information regarding how to perform some basic EPAF actions. Please note the EPAF submission deadlines shown. The following documents give you step-by-step guidance to perform functions such as placing an employee into a job assignment, ending an employee’s job assignment, or entering the correct labor distribution for a job assignment. If you have any questions about these documents, please email us.

EPAF Submission Deadlines

There are deadlines for submitting an EPAF for staff, service, faculty, and professional. Forms must be received in People and Culture Services with all the necessary approvals by the appropriate deadline in order for that assignment to be included in payroll for the period.

For Nonexempt staff, hourly professional, and service

11 a.m. on the last Friday of the biweekly pay period.

For faculty, professional, and exempt staff

Due in the Pre-Approval HR queue by the Friday immediately following the effective date.

 

Below are the links to various resources available to assist you in entering faculty assignment EPAFs. Faculty EPAFs are paid on the biweekly pay schedule during the academic year (typically falls between August and May). If you need to pay a faculty employee outside the academic year, please see Summer Assignments for instructions. Deadline for faculty EPAFs: need to be in the Pre-Approval HR queue by the Friday immediately following the effective date.

  • MTH 102 (PDF) – Contract Assignment – used for returning faculty semester primary assignments (F4 or F5).
  • MTH 107 (PDF) – Secondary Job - Additional Compensation with Labor - used when assigning a secondary job to a current faculty employee (F1, F2, F3, F4, F5, TF, or F6).  Link provides instructions on selecting and entering the correct MTH 107 EPAF.
  • MTH 107 (PDF) – Primary Job - Additional Compensation with Labor – instructions for assigning a temporary job (TF e-class) when it is the employee’s only (primary) job.
  • ADM 301 (PDF) - Change in Labor – used to change the FOAP a position is charged to.
  • ADM 500 (PDF) - Change of Supervisor or Org – used to change a supervisor and/or timesheet org.

Professional employees require an Professional Personnel Addendum to Employment Contract to be submitted with the EPAF.   • MTH 115 Primary Job (PDF) - Contract Assignment Renewal – used for professional employees returning to the same professional fiscal year contract position they were in the previous year (P3 & P4).
  • MTH 107 Secondary Job (PDF) - Additional Compensation with Labor - used when assigning a secondary job to a current professional/exempt staff employee. For professional employees - an Addendum to Contract for Faculty/Professional (Link to PDF) must be submitted along with the EPAF.  Link provides instructions on selecting and entering the correct MTH 107 EPAF.
  • MTH 107 Primary Job (PDF) - Additional Compensation with Labor – instructions for assigning a temporary job (TM e-class) when it is the employee’s only (primary) job.

All new non-exempt staff and service hires are entered through HR-TMS. DEADLINE for non-exempt staff and service: by 11:00 a.m. on the last Friday of the biweekly pay period.

  • BWK 103 (PDF)—Add or Reactivate a Secondary Job - used when you assign or reactivate a secondary job for a current staff employee. A Request for Compensation for Extra Work/Staff and Service (PDF) form must be submitted along with the BWK 103 EPAF.
  • BWK 115 (PDF)—Temporary Job Assignment Renewals (TB) - used to extend a temporary assignment.
  • BWK 201 (PDF)—Change Base Pay – used for shift changes only.
  • ADM 301 (PDF)—Change in Labor – used to change the FOAP a position is charged to.
  • ADM 400 (PDF)—End Job Assignment – used for service employees to end a job assignment.
  • ADM 500 (PDF)—Change of Supervisor or Org – used to change a supervisor and/or timesheet org number.

Use only summer position numbers (F6 e-class) for faculty payments that fall outside of the main academic year.  Faculty summer position numbers (F6) can be found On the Summer Position Numbers Chart.

Professional employees should continue to use TM position numbers for summer assignments.  Professional employees also require an Professional Personnel Addendum to Employment Contract to be submitted with the EPAF.
