Lost Checks

CURRENT/FORMER STUDENTS

If you would like to take action on a lost or outstanding check, please complete the Lost Check Re-Issuance Form.  The form will display any outstanding checks at the time you access the form.

Current and former students may enroll in direct deposit by accessing our secure website and entering the appropriate information.

VENDORS AND PARENT REFUNDS

If you would like to take action on a lost or outstanding check, please complete the Lost Check Re-Issuance Form for Vendors and Parent Refunds.  Please have your check number ready prior to completing the form; if you need assistance with identifying the check number, please contact our office.

Vendors may enroll in ACH for payments by completing the ACH Enrollment Form and returning it to our office. Unfortunately, due to system limitations, parent refunds may only be issued in the form of a check.
