Forms for Doctoral Students
Please refer to the dates and deadlines section for important Graduate School deadlines. In order to receive your degree, you will need to apply for graduation by the deadline, even if you do not plan to attend the ceremony.
Your department and advisory committee are responsible for determining the manual of style for writing a dissertation; however, the Graduate School determines the proper formatting and submission procedures for the product. Make sure your project follows the formatting and submission requirements for a dissertation and submit your project correctly.
Students should not register for DISS 799 (the dissertation course) until they have received notification that the Graduate School has approved their admission to candidacy. Please speak to your advisor about registration options the term you plan to propose.
For more information on your final product requirements, see the Graduate Catalog.
Below are forms you can complete online, which is the preferred method. If you do not see the form you need, please check the paper form listing below to print and return a paper copy to the Graduate School.
