About Our Office

The Office of Student Conduct provides the following services for the University community:

  • administers the Code of Student Rights and Responsibilities
  • responds to allegations of misconduct
  • provides support to the Associate Provost in grade appeals and academic misconduct procedures
  • helps students understand and navigate other university policies and procedures
  • oversees compliance with the Clery Act publishing annual security and fire safety reports for students, employees, and the general public.

Code of Student Rights and Responsibilities

The Code informs the community of the University's general behavioral expectations and outlines the procedures used to respond to complaints.

As a member of the Ball State community, it is important that you familiarize yourself with the various policies and procedures for the University community.

Annual Security and Fire Safety Reports

In accordance with the Clery Act, the Office of Student Conduct provides an annual security report and fire safety report. Find more on these reports or submit a Clery Report.

Incident Reporting

Please select the appropriate form below to begin your referral. If there is an emergency situation, please call 9-1-1 or University Police at 765-285-1111. Do not use the forms below to report events that present an immediate threat to health or safety.

Faculty who observe patterns of academic behavior in an individual student that may endanger their academic success are encouraged to complete this referral. This includes concerns with attendance, academic performance/preparation, etc.

The bias incident response team offers a supportive process students can use to report incidents involving bias. Students who may have been the target or witness to a bias incident may submit a report which will be reviewed by a member of the Bias Incident Response Team (BIRT). A team member will contact the student to discuss the incident further.

Faculty, staff, and students may refer Ball State community members who are experiencing crisis, displaying odd or unusual behaviors, or engaging in other behaviors that may be perceived as being harmful to themselves and/or others.

When appropriate, this information will be shared with the Ball State Counseling Center.

Silent Witness is an online program that allows users to confidentially report information about criminal activity on campus.

This form should not be used to report crimes in progress or emergencies and cannot be used to file formal police reports.

Members of the campus community have the opportunity to report issues that they believe may be a violation of the Code of Student Rights and Responsibilities, including but not limited to classroom disruption, disorderly conduct, and general student behavior reports.

The Office of Student Conduct will investigate reports to determine appropriate action.

Members of the campus community may refer incidents or concerns involving Ball State registered student organizations, including Greek letter organizations, for review of violations to the Student Code.
