Eligible employees who have received their COVID-19 vaccination are eligible for an incentive of either a one-time, $100 reduction on their health care premium or four (4) hours of paid time off (to be used by December 31, 2021). To qualify, employees must be vaccinated against COVID-19 no later than August 1, 2021, so that you are fully vaccinated prior to the beginning of the fall semester. Temporary, casual, and student employees (including graduate assistants) are not eligible to participate in the program.

In addition, beginning June 23 through August 4, 2021, four employees per week will randomly be selected to receive:
- One $100 bookstore voucher
- One $100 Tech Store voucher
- One $100 general parking permit
- $100 in dining cash
Additionally, for the drawing on July 7, 2021 we will offer three more one-time incentives in addition to those listed above that include the following for vaccinated employees:
- One all-access parking permit (valued at $650)
- One garage parking permit (valued at $469)
- A culinary class with Jason Reynolds, assistant director for University Catering
On August 4, 2021, we will offer three more one-time incentives in addition to those listed above for three vaccinated employees who will each win $500 in Tech Store vouchers.
Employees who participate in the University’s health care plan, but elect the paid time off option (to be used by December 31, 2021), will be required to use all accrued vacation/PTO and sick leave balances prior to using the four (4) hours of incentive time off. In addition, the time must be used by December 31, 2021, or it will be forfeited.
If you and your spouse are both employed by Ball State University and both enrolled in the same family coverage under a BSU health plan, then the spouse enrolled as primary can make a special marriage election to receive a $100 health care premium deduction for each vaccinated spouse, if both spouses are in benefits-eligible positions and the spouse not holding the health plan consents.
In order to apply for the voluntary incentive, please complete the form and verify that you are fully vaccinated. The form must be submitted no later than August 31, 2021.
