Employee COVID-19 Response Plan for Summer 2021

Self-Report Positive Results

Students and employees who received positive test results from any provider other than the University's Student Health Center or IU Health must self-report that they have tested positive through the Ball State COVID-19 self-reporting form. (Those who are tested at the Student Health Center do not need to complete this form.) Complete the form.

Testing and Contact Tracing FAQs

Ball State University has developed procedures in accordance with the CDC and Indiana State Department Of Health guidelines for testing, as well as procedures for the isolation, quarantine, and contact tracing of individuals who test positive for COVID-19. Learn more.

Employee Health Screening and Self-Certification Assessment

An employee who has been authorized to return to work on campus will be required to complete a self-certification form prior to returning. Essential employees who have continued working on campus will also be required to promptly complete the form. Complete the assessment.

Telework and Flexible Work Arrangements

Face Mask Information and Toolkit

Ball State is encouraging socially responsible behavior on campus and in the community, which includes requirements for face masks. Learn more.

Testing Sites

Testing is available on-campus at the Student Health Center.

Alternatively, the Indiana Department of Health provides a map of testing sites around the state. In Muncie, there is an ISDH/Optum Test Site at 401 N Country Club Road. View the map.

Face Mask Signs

Download and print a sign asking others to wear a mask or shield, then post it outside classrooms and around your work space to show that you care. We have two signs, depending on your needs: one for locations were a face mask is recommended and another for where one is required. Learn more.
