When prescribed by a physician, diabetic education will be covered at 100%, with no limit on the number of education sessions you may attend, provided the education is given by a Certified Diabetic Educator.

When prescribed by a physician, asthma education will be covered at a 100%, with no limits on the number of education sessions you may attend, provided the education is given by a Certified Asthma Educator.

When prescribed by a physician for a patient with hyperlipidemia, or hypertension, or diabetes, or pre-diabetes, nutrition counseling will covered at 100%, with no limit on the number of education session you may attend, provided the education is given by a Registered Dietitian.

All of these benefits will not be subject to deductible, except when required by law, such as in the High Deductible HAS Qualified PPO.  Participants in the High Deductible HSA Qualified PPO should remember to file claims for these services in order to count the expense toward meeting the deductible of their plan.  The above enhancements to the plan will be effective on July 1, 2008.
