Video Equipment Checkout
The Teleplex Equipment Checkout provides Faculty, Staff and Students with various video equipment such as digital video cameras, lighting, audio equipment, tripods, and all video accessories that are utilized in a professional location production shoot.

To ensure that all patrons have a chance to check out items the following policy has been adopted:

POLICY

Faculty/Professional Staff and BSU students may checkout equipment, however; BSU students must meet the following requirements:

1. Must be enrolled during current semester.

2. Must  have "STUDENT AUTHORIZATION for EQUIPMENT CHECK-OUT"  form signed by the faculty for whom the project is for. (Forms are in BC194)

Equipment use is restricted to University Related projects ONLY; it is not for personal endeavors.



Reserving Equipment:

Students cannot reserve equipment until the "STUDENT AUTHORIZATION for EQUIPMENT CHECK-OUT"   form has been completed and you must present your BSU ID card when turning in form.
  • You must have your Ball State ID card with you to reserve equipment.
  • Reservations must be done in person; reservations will not be taken by phone.
  • Students can make reservations up to one week in advance.
  • Reserving equipment for others IS NOT PERMITTED.


How long can you keep the equipment:
  • 24 hours….weekends counts as a 24-hour period.
  • All equipment checked out over a weekend is due by 11am on Monday.


Picking-Up Equipment:

Picking up equipment for other students IS NOT PERMITTED

Have your Ball State ID card with you to pick-up equipment

When you are checking out equipment, make sure everything you need is there and functioning before you leave. You are responsible for any damages or loss to the equipment at the time of return. In order to avoid any blame; make sure nothing is wrong with the gear before leaving. If you don't take time to check every piece of equipment, anything that malfunctions, damage, and loss will be your responsibility, even though you might have had nothing to do with it



Late Picking-Up Equipment:
Equipment reservations will be cancelled after1 hour from your scheduled time for pick-up; unless you call in advance to change your pick-up time.



Returning Equipment:
  • Wait while the lab assistants check and test the equipment; make sure you allow enough time.
  • Every item that you checked out must be present and in working order. This includes the careful packing of all cables and accessories.
  • Equipment failure must be reported upon return to the lab assistants.
  • Items discovered missing or damaged, you will be held accountable; you must replace it before reserving or checking out any additional equipment.
  • After everything has been checked and approved, you must sign the equipment back in.

Late Return Policy:

  • First time late, there is no penalty.
  • Second time late, you will be banned from equipment check-out for the remainder of the semester.

Liabilities:

  • The USER is held liable to pay all costs for replacement/repair of equipment due to misuse, neglect, lost and/or stolen; you will not be permitted to check out equipment until financial restitution has been made.
  • You will not be responsible for normal wear and aging of equipment; and unreported damages will be investigated.

The equipment for checkout was purchased to enable faculty and students to enhance and further the academic experience. You are borrowing equipment that others wish to use as well.  Abuse of this privilege by using the equipment improperly, by not returning it on time will result in the suspension of your privilege to borrow equipment.
