Copies of your medical records are available with a signed consent for release and payment of a copy fee. Copies are available for pickup usually within 3 working days. Picture identification is required to pick up your copies.
Note: If you are under 18 years of age, Indiana law requires that consent be obtained from a parent or legal guardian for medical care. Information regarding patients who are under 18 years of age can be released to a parent or guardian unless it is specifically protected by law.
The Health Center follows the law with regard to retention of old records. Unfortunately, the volume of records generated over a period of many years makes it impractical to retain records indefinitely. We encourage students to obtain copies of their records when they leave school for their own personal benefit. We retain records for 7 years after our last date of contact with a student, and then the records are destroyed. If it has been longer than 7 years since you graduated and left Ball State, we will not be able to find old records for you.
