When is the online SEA form available?

It is available for use twenty-four hours a day. During peak periods of Web-server use (such as 9 to 11 a.m. and 1:30 to 3:30 p.m.), response time may be slow. You might prefer to use the form early in the morning or late in the afternoon.

What is the Web address for the online SEA form?

Go to http://www.bsu.edu/formblaster to access Formblaster and the online SEA form. You may want to bookmark this address on your Web browser.

Who can access this form?

Only full-time university employees can access the form.

What's the difference between "Add a student" and "Change account number"?

Use "Add a student" to hire a new student or add a current student to an additional account number in your department. Use "Change account number" to drop a current student from one account number in your department and add him or her to another account number in your department in one transaction.

What do I do with the yellow student-employment-referral form?

Refer to it when you complete an online SEA form. Do not send it to the Career Center. You may discard it or keep it for your records.

After adding a student, do I need to request time sheets?

If your department uses the Kronos time-keeping system, contact Payroll to have your employee added. Otherwise, Payroll will automatically generate time sheets and send them to you in campus mail.

My department increased the base wage rate for a student position.  How do I get students already working in that position up to the new hourly rate?

Using the "Base Wage Rate Increase" option, enter the job number, the student's BSU I.D. number, and the account number.  FormBlaster will pull the new hourly rate from Cardinal ViewJobs.  You must have completed a Student Employment Change in Base Rate Request form and submitted it to the Career Center before attempting to process any SEAs.  Base rates above the $5.85 an hour minimum wage must be approved by your department chair or director and the appropriate vice president.

How do I process longevity increases?

Longevity increases are available at specified times of the academic year and dependent upon budgetary constraints of the department.  Using the "Longevity Pay Increase" option on the SEA, enter the job number, the student's BSU I.D. number, and the account number.  Use the "Increase Amount" pull-down box to select a 5-, 10-, 15-, 20- or 25-cent-an-hour increase.  Once you submit this form, it is routed to your budget head (department chair or director) for approval.  Longevity increases will be accepted only at the end of each semester and will become effective with the first full pay period of the next term. The time frame in which longevity increases are accepted and processed is announced at the end of each semester.  If you do not see the "Longevity Pay Increase" option on the SEA, longevity increases are not available.

What if I need to add a graduate assistant, doctoral assistant, undergraduate fellow, or resident assistant to student payroll?

If you try to complete an online SEA form for one of these students, you will get a red alert message. Do not use the online SEA form. Please complete a paper SEA form and send it to the Career Center.

I'm hiring a student for a graduate student position or an on-campus internship position at a special pay rate. Where do I enter the special pay rate?

Enter the requested hourly pay rate in the "Comments" field.

After submitting an online SEA form, what if I discover an error or change my mind?

Contact John Knox (285-2432, jrknox@bsu.edu) as soon as possible. He can delete the transaction.

Can I view the SEA forms that I have submitted?

Yes. To access your Formblaster home page, go to  http://www.bsu.edu/formblaster or click "Back to Formblaster!" (near the bottom of each SEA screen). You can review your pending and completed SEA forms or use the "Search Formblaster" function to find a particular form.

How do I log out of the online SEA form?

To log out, close your Web browser. This will prevent someone else from using your username and password. You do not need to shut down your PC.
