Doctor of Audiology Application Information
Application Deadline

All application materials must be received by February 1 to be considered for fall semester admission. Applications received after the deadline will only be considered if space remains in the program.

Admissions decisions are generally made in late February or early March, although outstanding candidates may be admitted early. Applicants will be notified of the admission decisions by e-mail and mail sent to the addresses indicated on your application.  (Please keep us informed of any changes to your e-mail or mailing address.)

All applicants must apply to and be accepted by the Graduate School and the Department of Speech Pathology and Audiology. The separate application procedures for each are outlined below.

Materials for the Graduate School (to be sent directly to the Graduate School)

  • Application Form
    Complete the Graduate Application form.  If applying online, include a printed copy of the completed application with the materials sent to the AuD Admissions Committee.

    In print (PDF). Detailed instructions for completing the Graduate Application form are given on the reverse side of the application. Mail the original completed application to the Graduate School. A copy of the completed application must be included with the materials sent to the AuD Admissions Committee.
  • Application Fee
    The application fee, listed on the application form, is to be sent directly to the Graduate School along with the completed application form. Applications cannot be considered without payment of the application fee.
  • Transcripts
    Request that official transcripts of all college/university course work, including any graduate work, be sent directly to the Graduate School from the issuing institution. Original transcripts must be provided from all institutions attended even if transfer courses from other universities are listed on other transcripts. NOTE:  Effective November 2007, a second set of transcripts does not need to be sent to the AuD Admissions Committee.  This is a new policy and may not be reflected in the application information mailed to applicants prior to this date.
  • GRE Scores
    Request that GRE scores be sent directly to the Graduate School. 


Materials for the AuD Admissions Committee
(to be sent directly to the AuD Admissions Committee--all together in one mailing)

  • Statement of Purpose
    In the statement of purpose, each applicant is asked to provide additional information to the AuD Admissions Committee about audiology as his or her career choice, long-term career objectives, and educational and experiential preparation for an AuD program. The statement must be typed and should be no more than two single-spaced pages.
  • Supplemental Information Form
    Complete the Supplemental Information (PDF) form. This form may be handwritten.
  • Documentation of Clinical Observation/Practicum Hours
    Many students from communication disorders programs will have acquired observation or practicum hours.  Although these are not required, students who have acquired such hours should provide official documentation of these experiences.  Documentation of these experiences must be signed by the person responsible for certifying clinical hours. If additional observation or practicum hours are obtained before starting at Ball State, admitted students will also need to submit updated documentation that includes these hours. Requirements for these hours are listed here.
  • Recommendations
    Each application must be accompanied by three recommendations from people who know the applicant.  Recommendations can be in the form of letters or by using the recommendation form (PDF). Each person should be familiar with the applicant's academic and/or clinical work and be able to comment on the potential for success in a doctoratal program. At least one reference must be familiar with the applicant's academic work and, if applicable, at least one should be familiar with the clinical work.
    All three recommendations must be included with the application materials sent to the AuD Admissions Committee. If the person writing the recommendation prefers that it not be read by the applicant, then it may be provided to the applicant in a in a sealed envelope signed across the flap.
  • Copy of Completed Graduate School Application
    Include a copy of the completed application to the Graduate School. If the application was submitted online, include a printed a copy of the completed application.
  • Transcripts
    See note above - as of November 2007, only one set of transcripts is to be provided to the Graduate School.  A second set no longer needs to be provided to the AuD Admissions Committee.
  • GRE Scores
    Applicants must include a copy of GRE scores. These do not need to be official copies; a photocopy is acceptable.

On-campus Interview and Campus Visit
The on-campus interview is critical to the application process. The interview gives the AuD Admissions Committee the opportunity to further assess each applicant's potential for success in the AuD program.  It also provides each applicant with the opportunity to verify that our program is compatible with his or her career objectives. Additional information about the on-campus interview will be provided after all application materials have been received.
