Social Work Major Admissions Contract

All students accepted formally into the social work major are asked to read and sign the contract below when meeting with the Chair for the final admissions interview.  In preparation, students are expected to have reviewed the Code of Ethics at http://www.socialworkers.org/pubs/code/code.asp

I agree to the following:

I have read and am familiar with the NASW Code of Ethics and I agree to  adhere to its principles and tenets.

I will continue my professional development through regular use of university andcommunity resources;

I will schedule advising sessions as needed with my Faculty Advisor to discuss my academic and professional development;

I give my Faculty Advisor permission to discuss my progress in the social work department with the Chair and other relevant university personnel.

I understand that, although I am admitted to the social work major, if my academic and professional development is deemed unsatisfactory by the social work faculty, the Department of Social Work has the right and responsibility to request reassessment of my suitability for the major in Social Work.   The social work faculty advisor will provide academic advising to assist with my learning and professional development process in order to meet the academic and professional standards of the major.

Furthermore, by my signature placed below, I certify that the information contained in the application to the Bachelor of Social Work degree program is correct to the best of my knowledge, and understand that falsification or omissions in this application in any detail is grounds for disqualification from further consideration for formal admission or for dismissal from the social work major at the time the omission or falsification is discovered. 

I further certify that I have not been convicted of, or have charges pending for, any of the following offences:

• Felony
• Felony assault / assault
• Drug related criminal activity
• Theft
• Domestic violence
• Child Abuse
• Murder / Manslaughter
• Felony DUI (bodily injury)
• Stalking, Sexual assault /rape

I agree to immediately notify the Department Chair should I be charged with or convicted of a felony or misdemeanor, or any criminal offense listed above after being formally admitted to the major.
