NOTICE TO ALL STUDENTS
The Family Educational Rights and Privacy Act (FERPA) affords students certain rights with respect to their education records. These rights are:
1. The right to inspect and review the student's education records within 45 days of the day the university receives a request for access. Students should submit to the vice president for student affairs written requests that identify the record(s) they wish to inspect. The vice president for student affairs and enrollment management will make arrangements for access and notify the student of the time and place where the records may be inspected. If the records are not maintained by the vice president for student affairs and enrollment management, he or she shall advise the student of the correct official to whom the request should be addressed.
2. The right to request amendment of the student's education records if the student believes they are inaccurate or misleading. Students should write to the university official responsible for the record, clearly identify the part of the record they want changed, and specify why it is inaccurate or misleading. If the university decides not to amend the record as requested by the student, the university will notify the student of the decision and advise the student of his or her right to a hearing regarding the request for amendment. Additional information regarding the hearing procedures will be provided to the student when notified of the right to a hearing.
3. The right to consent to disclosures of personally identifiable information contained in the student's education records, except to the extent FERPA authorizes disclosure without consent. One exception, which permits disclosure without consent, is disclosure to school officials with legitimate educational interests. A school official is defined as person employed by the university in an administrative, supervisory, academic, or support staff position (including law enforcement and health staff); a person or company with whom the university has contracted (such as an attorney, auditory, or collection agent); a person serving on the Board of Trustees; a student serving on an official committee such as a disciplinary or grievance committee; or a person assisting another school official in performing his tasks. A school official has a legitimate educational interest if the official needs to review an education record in order to fulfill his or her professional responsibility. Upon request, the university discloses education records without consent to officials of another school in which a student seeks or intends to enroll.
4. The right to file a complaint with the U.S. Department of Education concerning alleged failures by the university to comply with the requirements of FERPA. The name and address of the office that administers FERPA is: Family Policy Compliance Office, U.S. Department of Education, 400 Maryland Avenue, S.W., Washington, D.C. 20202-5901.
The Records Policy is available under "Code of Student Rights and Responsibilities" at www.bsu.edu/sa/srcs/studentcode/, or a paper copy of the policy may be obtained from the Office of Student Rights and Community Standards in the Student Center, room 224.
Under the provisions of the Family Education Rights and Privacy Act, a student has the right to withhold the disclosure of any or all "Directory Information." The items that a student can request to be withheld from the BSU Directory, news media, or any other requests are: name; address (local, home); sex, telephone listing (local, home), date and place of birth; major field of study; class or years of study; class schedule; participation in officially recognized activities and sports; weight and height if a member of an athletic team; dates of attendance; degrees and awards received; and the most recent previous educational institution attended. Request forms to prohibit the release of information are available in the Office of the Registrar, Lucina Hall, room B43.
Please consider very carefully the consequences of any decision by you to withhold any category of "Directory Information." Should you decide to inform the institution not to release any or all of the "Directory Information," any future requests for such information from noninstitutional persons or organizations will be refused.
If your local or home address has changed, you need to submit a Change of Address Form to the registrar's office, Lucina Hall, room B43.
