Name Change Request
Your records at Ball State University are built from your initial admissions to the university in one of the following offices: Undergraduate Admissions, Graduate Admissions, International Programs or Extended Education. In order to keep all your records updated and maintained with the correct identifier under your legal name you will need to provide the Registrar's Office with appropriate information.
A name change request allows the student or former student to modify their academic record to reflect a change in his/her legal name.
To protect the integrity of your Academic Record, we require your written authorization and documentary proof of your name change. Your written authorization should include your Ball State ID Number or your Social Security Number, all former names, the year and term you last attended at BSU, a telephone number where you may be reached during the workday, your complete new name, and your signature authorizing the change. Please use the Name Change Request form located on this page to authorize us to change your name on your Academic Record.
Documentary proof of your name change should include a driver's license, marriage license, divorce decree or other legal document. If returning this form by mail or fax, copies of legal documents should be included with this form.
Please note: If you are submitting a copy of divorce papers, please do not include any provisions other than that ordering the name change. That is, include only the jurisdiction (court, county,state), the name of the parties, the name change provision, signature and date of the judge or clerk of the court.
Anyone who is on the the Ball State University payroll and changing their name, should contact Payroll and Employee Benefits at the Administration Bldg. AD 29, 765-285-8461, to insure that their changes are reflected on their pay.
