SPORTS PERFORMANCE MASTERS PROGRAM

APPLICATION CHECK SHEET

To be considered for the masters degree in Sports Performance at Ball State University, the following items must be completed by February 15.  Please use this check sheet to insure that you have completed all of the necessary application steps.

MASTERS PROGRAM APPLICATION PROCESS: all of these items must be submitted before your application will be reviewed.

_____1.  Masters application form: this form supplies background information on each candidate.

_____2.  One copy of official transcript(s) stating degree earned. Evaluation of your academic preparation involves examining both your grades and the courses.

_____3.  Curriculum vita:  this summarizes all of your educational and work experiences.  It should be complete, but brief.

_____4.  Three letters of recommendation: more than three letters can be sent, but they should come from individuals who can judge your potential as a graduate student.

_____5.  GRE scores: if you cannot take the exams prior to submission or have not received your scores, indicate such in your application and give a possible receipt date.  TOEFL scores are required of all International students.

NOTE:  It is to your advantage to have these available by the Feb. 15 deadline.

GRADUATE SCHOOL APPLICATION PROCESS:  in addition to applying to our program, all students must apply and be accepted into the Graduate School of Ball State University.  To be considered, the following items must be submitted.  PLEASE NOTE THAT A BACCALAUREATE DEGREE OR EQUIVALENT IS  REQUIRED FOR ADMISSION.

_____2.     Two official copies of your transcript(s)

_____3.     GRE scores
