The following items must be completed by January 15. Please use this check sheet to insure that you have completed all the necessary application steps. PLEASE NOTE THAT YOU MUST HAVE A MASTERS DEGREE TO ENTER THE PROGRAM.
Ph.D. PROGRAM APPLICATION PROCESS: all of these items must be submitted before your application will be reviewed.
____1. Ph.D. application form: the first page supplies background information on each candidate. The second page will be used to evaluate your ability to write and organize your thoughts, and supplies information concerning your research experience.
____2. One copy of official transcript(s): evaluation of your academic preparation involves examining both your grades and the courses you have taken. Your GPA must be 3.0 or higher on a 4-point system.
____3. Curriculum vita: this summarizes all of your educational and work experiences. It should be complete, but brief.
____4. Three letters of recommendation: more than three letters can be sent, but they should come from individuals who can judge your potential as a graduate student.
____5. GRE scores: if you cannot take the exams prior to submission or have not received your scores, indicate such in your application and give a possible receipt date.
____6. Philosophical statement: motivation for attending graduate school.

GRADUATE SCHOOL APPLICATION PROCESS: in addition to applying to our program, all students must apply and be accepted into the Graduate School of Ball State University. To be accepted, the following items must be submitted:
