Information on Health Care Plans
  1. Do I need an appointment to make a change to my insurance?
    No, you may come to PEB anytime during regular business hours.
  2. What happens to my insurance if I leave the university?

  3. What are Key Benefit Administrators (KBA) telephone numbers?

  5. What is the age limit for dependent children to remain on the Health Care Plan?
    As long as the child is unmarried and a dependent of the employee they may remain on the plan until the end of the calendar year in which they turn 23.
  6. When can I make a change on my health care plan?
    Generally only during Open Enrollment, which is, April 15 – May 31 with the change effective on July 1.  Exception:  Some changes can be made within 31 days of a family status change.  You should contact PEB for specific information.
  7. What is the time limit for filing claims?
    Claims may be filed until the end of the calendar year following the date of service year.
  8. Are any specific prescription medications not covered by our plan?
    Any questions regarding prescription drug coverage with Anthem should be directed to the PEB office.
  9. I filed a prescription drug claim with KBA and it was returned. Why?
    Prescription drug coverage is with Anthem not KBA.  If you did not use your Anthem card at the time the prescription was filled, please complete an Anthem claim form.
  10. Why is Anthem trying to make me take a different prescription?
    Your pharmacist may suggest a generic product equivalent to the product prescribed.  However, this is only a suggestion and you do not have to change products.
  11. What are the deductibles with the BSU Health Care Plans?
    Medical  $200/person, $400/family maximum
    Dental $50/person, $100/family maximum
    Prescription $25/person, $50/family maximum

  12. What is the employee share of premium for the BSU Health Care Plans?
    Traditional Single $203.82/mth, Family $529.10/mth
    PPO Single $128.84/mth, Family $334.44/mth

  13. What is the Dental annual maximum benefit?
    It is $1,000/person in both Health Care Plans.
