Long Term Disability
Salary continuance coverage is ('base annual pay' x 103%)/12 x 60%.

Benefit

Employee's monthly disability benefit amount = (‘base pay' x 103% x 60%)/12
Employees are eligible for up to 24 months of benefit for an illness or injury that causes them to be disabled from doing their current occupation, as defined by CIGNA.
Employees are eligible for an extended benefit until June 30th following their 66th birthday if they continue to qualify as disabled from any occupation.

Eligibility
Staff/Service employees are eligible after completing their 90-day probationary period.
Faculty/Professional employees are eligible their first day of work

Filing a Claim

PEB will send you an Early Notification Program brochure once you have been absent from work for a minimum of 4 weeks. (If we are properly notified of your absence.)
Application needs to be made within 90 days of your qualifying disability date
