Staff and Service Personnel Honorees'Deadline to Return Choice-of-Award Order Forms

Monday, December 15, 2008 -- Choice-of-award order forms must be returned.  Any honoree who does not return his or her order form by the due date will be presented with the award marked with an asterisk on the order form.

If You Are Eligible for Retirement:

Friday, January 30, 2009 - Employees eligible for retirement must complete the appropriate forms in Payroll and Employee Benefits on or before January 31 if they wish to be recognized at the 2008 recognition program. Employees who enroll  after the cutoff date will be recognized at the 2009 recognition program.

Staff Recognition and Retirement Dinner Tickets 

Honoree Complimentary Tickets Available:

Monday, March 2 - Friday, March 13, 2009, in the Office of University Human Resource Services in AD 350. 

  • Years of service honorees receive one (1) complimentary ticket.
  • Retirees receive two (2) complimentary tickets.  For additional tickets see "Ticket Sales."


NOTE:  If you plan to atend the award program, but will not be available to pick up your complimentary ticket during these two weeks (vacation, etc.), please contact Marta Stephens immediately at 285-1187.

Ticket Sale:
Monday, March 16 - Friday, March 27 (3 PM)  available at the cashiers' windows in the Office of Bursar and Loan Administration, Lucina Hall, during working hours (8 AM - 4:30 PM).  The cost per ticket: TBA.

Reservations for the Staff Recognition Dinner  will be accepted between March 2 - March 27 2009.   All reservations MUST be made in writing to mstephen@bsu.edu.   Please include the names of all guests.  Confirmation will be made in writing for ticket holders only.


Other Staff Award Recipients:

A. Jane Morton Award Nominee(s) may pick up his or her complimentary ticket between Monday, March 2 - Thursday, March 13, 2009 in the Office of  University  Human Resource Services in AD 350.  

Meritorious Service Award Recipients.  The four winners' names are announced on the evening of the recognition award program. NOTE:  The individual from each area who nominates the winner is responsible for (1) ensuring the winner from their area remains unaware that he or she is the recipient of the award, (2)  purchasing the award recipient's ticket, and (3) making sure the recipient attends the recognition program without arousing his or her suspicion of the award.

Service Personnel Recognition and Retirement Program

Service Recognition and Retirement Dinner Tickets available in AD 350 on Monday, March 2, 2009, on a first come first serve basis.   

Honoree Complimentary Tickets Available:

Order forms will be mailed to service personnel honorees by February 2.  Friday, February 13, 2009, is the deadline for honorees to return their ticket reservation order form. It is strongly suggested honorees return this form by February 13 to ensure their tickets are set aside.  Limit: 10 tickets per family (honorees).

  • Reserved honorees' tickets will be ready to be picked up.
  • Remaining tickets will be disbursed to non-honorees on a first come, first serve basis with a limit of 5 tickets per family based on availability.


Cancellations: 
Honoree ticket holders who are unable to attend the program on May 15 are asked to please contact Marta Stephens at 285-1187.   All tickets must be returned to UHRS.

Seating reservations available ONLY for employees being recognized for 30 or more years of service and those being recognized for retirement in 2009. All reservations must be made in writing to: Marta Stephens, Ball State University, University Human Resource Services, AD 350, Muncie, IN 47306

NOTE:  CHILDREN UNDER TWO DO NOT NEED A TICKET TO THE MAY 15 EVENT.
