FCS Alumni Awards Online Nomination Form

All nominees must be an alumnus of the Ball State University Department of Family and Consumer Sciences (formerly Home Economics). Current members of the FCS Alumni Association Board or the Ball State University Alumni Council are not eligible for an award until their term of service is complete.

*FCS Emerging Professional Award of Merit--Nominee has demonstrated an excellent potential for future success in chosen field and has exhibited a history of success within the FCS department.

*FCS Award of Achievement--Nominee is in a position of responsibility and has demonstrated a history of success (promotions, awards, recognitions, etc.) within the FCS department and/or academe.

*FCS Award of Distinction--Nominee holds or has held a position of distinction in education, business, government, or academe; demonstrates outstanding success in his or her field, related to family and consumer sciences; demonstrates loyalty to, support of, and leadership on behalf of the FCS department; is recognized as a civic leader through volunteer service to his or her community; and displays extraordinary service to human kind.

Award nominations are accepted on an ongoing basis. Awards are presented at the bi-annual Family and Consumer Sciences Spring Alumni Day. Nominations must be received by December 1 to be considered for the succeeding spring’s awards presentation. If you have supporting materials to submit (resume, press clippings, etc.), send a print out of this completed form, along with the supporting materials to: FCS Alumni Awards Program, Alumni Association, Ball State University, Muncie, IN 47306-0075.
