Alumni - CAP Alumni Awards Online Nomination Form

Each year during Alumni Day, the Governing Board of the CAP Alumni Society recognizes the professional accomplishments of alumni with two categories of awards. The Distinguished Alumnus Award is given to selected alumni with 15 years or more of professional experience who have achieved positions of influence and national or international reputations in their fields. In addition, Awards of Outstanding Achievement are made to selected alumni who are making outstanding contributions to their profession and to society. There is a two part process to the alumni award program. Nominations may be made by any alumni on behalf of another or for themselves. This year, NOMINATIONS ARE DUE by Friday, March 30, 2012 to Lori Pence in the Dean’s Office of CAP.

All nominees will be notified in writing and are then invited to electronically submit a final award application which is a comprehensive summary of their professional accomplishments, letters of support and a letter highlighting their professional contributions. Any additional supporting materials are due no later than Wednesday, May 30, 2012, for review by a jury of CAP alumni peers. Winners will be notified in July 2012 and the awards presented in October 2012 at the Alumni Awards Banquet. Please consider taking a moment to nominate worthy candidates for these prestigious awards.
