To make an appointment, print out the clinic application packet and complete all three parts. Then mail the application to:

Upon receipt of your application, a qualified clinician will call you and make an appointment for your intake. At an intake session, you will have an opportunity to discuss the presenting problem, choose among options for clinic services, and arrange for payment.
Applications are processed in the order received. There is usually a short wait (up to 90 days) before clients are scheduled for their intake appointment.
