If you have not provided the University with your SSN, a Form W-9S will be mailed to your local address by November 15^th.  If you did not receive a form please go to www.irs.gov/pub/irs-pdf/fw9s.pdf  to print the form.  Forms are also available at the Office of Bursar and Loan Administration, LU B31.  December 1^st is the deadline for the completed forms to be returned to Ball State University, Office of Bursar and Loan Administration, Muncie, IN  47306-0725.
