Form 1098-T
Form 1098-T is automatically mailed to the last known home address (or local address if home address was not provided) for every student who took credit classes during the calendar year. The IRS requires Form 1098-T be postmarked by January 31st of the next calendar year. 

Current Students

  • You may access your Form 1098-T for the previous tax year by using your BSU computer user id and password (same as what is used for other web applications, i.e. grades, change of address, course request).  If you are a current student and do not have a user id and password, you must visit RB 165 in person and present your BSU ID to obtain your BSU computer user id & password. The same procedure applies if you have lost or forgotten your password. You must visit RB 165 in person and present your BSU ID.

Former Students

  • If you are not enrolled in the current Spring semester, if it is after February 15th, and you did not receive your Form 1098-T for the previous tax year, please e-mail the following information to bursar@bsu.edu in order to request a duplicate.
    • your full name
    • student id number
    • address to which the statement needs to be mailed
    • daytime phone number at which you can be contacted if there are questions

Legal Guardians

  • If a student is currently enrolled, the student may access their Form 1098-T for the previous tax year after January 31st, by using their BSU computer user id and password (same as what is used for other web applications, i.e. grades, change of address, course request).  If you wish to access this information directly, your student will need to provide you the required computer user id and password.  Because this information is accessible online to current students, we expect you to obtain it from your student and will not honor requests for it to be mailed directly to you.
  • As the legal guardian of a former student, you may request a duplicate Form 1098-T be sent to you provided your guardianship can be verified.  If guardianship cannot be verified, the student must make the request. Please e-mail the following information to bursar@bsu.edu.
