Athletic Physical Reports and Athletic Transfer Reports are available for download below. Please review carefully the requirements of each.
Physical Report: The IHSAA Athletic Physical Form must be completed after May 1 of the year school participation begins in the fall. This must be delivered to the Athletic Office, Burris room 114, by hand or by mail. It must be complete with all required signatures - physician, participant, and parent.
Athletic Transfer Report: The IHSAA Athletic Transfer Form must be completed by any student (and parent) entering grades 10, 11, or 12 from another school. The student and parent must consult with the athletic director as soon as possible after admission to Burris is granted. This form is required if a student plans to participate in any sport during the school year.
