Notice Concerning Request for Social Security number: Ball State University is required by federal law to obtain Social Security numbers from individuals to whom it pays salaries, wages or other reportable compensation, from students who pay tuition and related expenses to the University, from students and prospective students who apply for or receive financial aid, and for certain other purposes as provided in federal laws or regulations.  If one of these requirements applies to this form or application, it will not be processed unless you provide your Social Security number to us.

In all other cases, you have the right to refuse to provide your Social Security number, if you so desire, and you will not be penalized for such refusal.  The University will not disclose your Social Security number to anyone who does not need access to the number for legitimate University purposes without your consent, except as required by applicable laws or regulations.
