Spring Semester 2008 Adherence Program
The goal of SPRINGO:

Sometimes the most difficult part of exercising regularly is incorporating it into your busy daily routine. Through participating in SPRINGO, you can improve your exercise consistency, challenge yourself to do more, and track your progress as we spring into a healthy new year!

THE RULES

The number in each bingo square corresponds to a day that the APFP is open in that month. By attending and completing a workout that day, you can check that day off on your Bingo card. You are responsible for keeping track of your own Bingo card.

For each bingo, you will receive one (1) raffle ticket.

These raffle tickets will be entered in the grand prize drawing at the end of the 3-month program. Therefore, the more bingos you get, the better your chance of winning the grand prize.

Each month, you can receive a maximum of 4 bingos. If you blackout your card for the month, you are not only receive the maximum of four (4) raffle tickets, but you will receive a medium prize for that month.

All participants will receive a small prize at the end of the SPRINGO program.

·  The only variations of bingo that will be counted are straight across, straight down, or diagonal. (No postage stamps, 4 corners, etc.)

·  There are four (4) "free" spaces each month. In order to receive them, the participant must:

·  1. Check their body weight.

·  2. Have their blood pressure checked by a GA.

·  3. Ask a GA for one new stretch to add to exercise program.

·  4. Discuss and set a goal for a healthy food change with a GA.

·  Each of these free spaces must be initialed by a GA.

·  All cards will be kept in a file box, which will be kept at the front desk for easy, quick access by participants and GA's.
