CAP Alumni Awards Online Nomination Form

These awards will be publicized yearly in reCAP, eCAP, and the Ball State Alumnus Magazine. Any alumnus of the College of Architecture and Planning is eligible for award consideration and anyone may submit a nomination.

CAP Distinguished Alumnus Award--The nominee, who must be an alumnus of Ball State University with 15 or more years of professional experience, should hold a position of eminence with a national/international reputation.
CAP Award of Outstanding Achievement--The nominee should hold a position of distinction, and should demonstrate outstanding success in his or her field. He or she should be making outstanding contributions to their profession and to society.

Award nominations are accepted on an ongoing basis, however, if you would like to be considered for the awards for the current year, please submit your materials no later than the first Friday in April. Awards are presented at the annual CAP Alumni Day each fall. Nominations will remain in consideration for three years. If you have supporting materials to submit (resume, press clippings, etc.), send a print out of this completed form, along with the supporting materials to: CAP Alumni Awards, College of Architecture and Planning, Ball State University, Muncie, IN 47306-0075.
