These high-quality self-adhesive labels come on an easy-to-store fold up sheet (45 labels to an 8 1/2 by 11 sheet).

Labels can also be used to identify personal belongings: books, luggage, CDs, videos.  Be creative!

To obtain  your labels, just use the SECURE ORDER FORM  and pay with a credit card.  Make sure that you provide all of the requested information.

If you prefer not to utilize the online method, the appropriate form may be downloaded and mailed to the Ball State Alumni Association along with payment.

Once the form has been downloaded, you may print, complete, and mail the application to us along with your check or money order made payable to the Ball State Alumni Association.   Upon receipt and processing,  you should receive your address labels in approximately two weeks.
